For more than a century, community outreach has been at the core of the San Antonio's mission. Today, we continue to demonstrate our commitment to improving health in surrounding communities and populations by providing organized and sustainable programs and services. We also conduct a comprehensive triennial Community Health Needs Assessment (CHNA) to further evaluate and prioritize community needs. This process culminates in the development of a three-year strategy and implementation plan. In addition to the comprehensive assessment conducted every three years, secondary data collected through the CHNA is updated annually to continue to identify and meet the needs of the population we serve. San Antonio believes in the power of collaboration and seeks alliances with other health and social service providers to develop community-based programs with defined goals and measurable outcomes. These partnerships help to leverage the community’s scarce resources to achieve the maximum benefit for its residents. Our primary service areas include the cities of Claremont, Chino, Fontana, Montclair, Ontario, Rancho Cucamonga, and Upland.
